Your Hospice Connection, hello my name is Helen Smith.
I am your clinical hospice liaison. I am here to help make the transition into our care as comfortable as possible for you and your family.
Your goals are my goals. Everyone's needs are different which is why I approach each one of our patients and families with a conversation about what matters most to you, not just a list of what we provide. My job is to then communicate and coordinate your goals for care to our compassionate hospice team.
Upon admission, our registered nurse will create a plan of care according to your needs and goals. These services may include:
>Coordination of medical equipment
>Medication Management
>Nursing and nursing aide visits
>Support through social work, counseling and chaplain services
